Assault

• At 2:17 p.m. Sunday, Juneau Police Department investigated a report of assault in the 200 block of Front Street.

False statements on resident commercial crewmember licenses

• On Monday, Juneau Alaska Wildlife Troopers issued a court summons to Joseph D. Hales, 26, of St. George, Utah, for two counts of false statements on resident commercial crewmember licenses for 2010 and 2011, false statements on a 2010 resident Juneau wetlands permit, false statements on 2010 resident deer harvest tags, and false statements on 2010 resident sport fish and hunting licenses. Investigation stemmed for an initial report of Hales failing to return his 2010 resident Juneau wetlands permit to the local Alaska Department of Fish and Game office. Hales falsely obtained all the resident applications when he did not meet Alaska resident requirements. Arraignment is set for 1:15 p.m., Dec. 29, in Juneau District Court.

Fire and medical

• At 1:50 a.m. Friday, Capital City Fire and Rescue responded to a fire alarm sounded from burnt food on Willoughby Avenue.

• At 7:35 a.m. Friday, basic life support care was provided to a woman on Glacier Highway, and she was taken to Bartlett Regional Hospital.

• At 11:03 a.m. Friday, EMS provided care to a man on Commercial Boulevard, and he was taken to BRH.

• At 12:45 p.m. Friday, CCFR units were dispatched to Vintage Boulevard, then cancelled en route.

• At 5:39 p.m. Friday, advanced life support care was provided to a woman on Glacier Highway, and she was taken to BRH.

• At 9:30 p.m. Friday, EMS provided care to a woman on Northwood Drive, and she was taken to BRH.

• At 12:43 a.m. Saturday, EMS provided care to a man on Glacier Highway, and he was taken to BRH.

• At 1:48 a.m. Saturday, advanced life support care was provided to a man on Franklin Street, and he was taken to BRH.

• At 4:21 a.m. Saturday, EMS provided care to a man on Glacier Highway, and he was taken to BRH.

• At 4:42 a.m. Saturday, a man on Hospital Drive was medevaced from BRH.

• At 6:28 a.m. Saturday, a woman on Hospital Drive was medevaced from BRH.

• At 3:56 p.m. Saturday, advanced life support care was provided to a woman on Behrends Avenue, and she was taken to BRH.

• At 9:18 p.m. Saturday, EMS provided care to a woman on Willoughby Avenue, and she was taken to BRH.

• At 10:17 p.m. Saturday, EMS provided care to a woman on Ninth Street, and she was taken to BRH.

• At 6:40 a.m. Sunday, EMS responded to a vehicle accident on Montana Creek Road and found no injuries. No transport was required.

• At 6:44 a.m. Sunday, EMS provided care to a man on Distin Avenue, and he was taken to BRH.

• At 8:15 a.m. Sunday, CCFR took a woman from BRH to a private residence.

• At 10:12 a.m. Sunday, basic life support care was provided to a woman on Foster Avenue, and she was taken to BRH.

• At 10:34 a.m. Sunday, basic life support care was provided to a woman on Wickersham Avenue, and she was taken to BRH.

• At 12:12 p.m. Sunday, EMS provided care to a woman on Willoughby Avenue, and she was taken to BRH.

• At 3:03 p.m. Sunday, EMS provided care to a woman Cinema Drive, and she was taken to BRH.

• At 6 p.m. Sunday, CCFR helped assist police or another agency, and no transport was required.

• At 6:30 p.m. Sunday, EMS provided care to a woman on Trafalgar Avenue, and she was taken to BRH.

• At 8:42 p.m. Sunday, advanced life support care was provided to a man on Teal Street, and he was taken to BRH.

• At 8:55 a.m. Monday, basic life support care was provided to a man on Nowell Avenue, and he was taken to BRH.

• At 10:21 a.m. Monday, EMS provided care to a woman on Great Western Street, and she was taken to BRH.

• At 10:31 a.m. Monday, basic life support care was provided to a man on Renninger Street, and he was taken to BRH.

• At 1:24 p.m. Monday, EMS provided care to a woman on Sesame Street, and she was taken to BRH.

• At 1:27 p.m. Monday, basic life support care was provided to a man on Decoy Boulevard, and he was taken to BRH.

• At 3:58 p.m. Monday, CCFR took a man from BRH to a private residence.

• At 5:53 p.m. Monday, a man on Glacier Highway declined care and transport.

• At 6:27 p.m. Monday, a woman was medevaced from BRH to Juneau International Airport.
