Conditions violation

• At 6:39 p.m. Monday, the Juneau Police Department investigated a possible DVO violation.

Driving without a license

• At 9:48 p.m. Monday, Joseph Lofstrom, 30, was cited for driving without a license on Spruce Street, and his vehicle was impounded.

False statement on a license application

• On Monday, Alaska Wildlife Troopers in Juneau issued Bryan D. Morris, 55, of Juneau, a $310 bailable citation out of Juneau District Court for obtaining a 2007 resident hunting license when he did not meet Alaska’s resident requirements.

• On Monday, Alaska Wildlife Troopers in Juneau issued Donald G. Lind, 66, of Juneau, a $310 bailable citation out of Juneau District Court for obtaining a resident 2011 sport fish license when he did not meet Alaska’s resident requirements.

Fire and medical

• At 1:27 a.m. Monday, Capital City Fire/Rescue was dispatched to Kanat-A Street for possible EMS for a female patient. No EMS or transport required.

• At 2:50 a.m. Monday, basic life support care was given to a woman on Birch Lane, and she was taken to BRH.

• At 4:27 a.m. Monday, CCFR was dispatched to Hospital Drive for a fire alarm. Alarm sounded due to cigarette smoke. Returned to service.

• At 4:45 a.m. Monday, basic life support care was given to a woman on Amalga Street, and she was taken to BRH.

• At 5:57 a.m. Monday, basic life support care was given to a child on Willoughby Avenue, and the child was taken to BRH.

• At 7:56 a.m. Monday, CCFR was dispatched to Hermit Street for a person trapped in an elevator. Elevator repair contacted, and no ENS or transport required. Returned to service.

• At 8:21 a.m. Monday, CCFR was dispatched to Renshaw Way for possible EMS, and no EMS or transport was required.

• At 8:49 a.m. Monday, advanced life support care was given to a man on Glacier Highway, and he was taken to BRH.

• At 9:01 a.m. Monday, advanced life support care was given to a woman on C Street, and she was taken to BRH.

• At 11:11 a.m. Monday, basic life support care was given to a woman on Shell Simmons Drive, and she was taken to BRH.

• At 11:54 a.m. Monday, advanced life support care was given to a man on Ninth Street, and he was taken to BRH.

• At 12:35 p.m. Monday, CCFR was dispatched to Third Street for an automatic fire alarm. No smoke or fire found. Alarm sounded due to maintenance. Returned to service.

• At 12:46 p.m. Monday, advanced life support care was given to a man on Whittier Street, and he was taken to BRH.

• At 2:32 p.m. Monday, CCFR provided transport for two infants and crew from BRH to Juneau International Airport for medevac.

• At 2:59 p.m. Monday, CCFR provided transport for a female patient and crew from BRH to JIA for medevac.

• At 3:15 p.m. Monday, advanced life support care was given to a man on Commercial Boulevard, and he was taken to BRH.

• At 3:21 p.m. Monday, CCFR was dispatched to a vehicle accident without injuries, no location given. CCFR assisted with driver extrication. No EMS or transport required.

• At 5:13 p.m. Monday, advanced life support care was given to a man on Alder Street, and he was taken to BRH.

• At 6:36 p.m. Monday, CCFR was dispatched to Glacier Highway for a possible fire. No smoke or fire found. Electrical problem was found. Suggested contact electrician. Returned to service.

• At 8:43 p.m. Monday, advanced life support care was given to a woman on Northwood Drive, and she was taken to BRH.

Motor vehicle crash

• At 3:53 p.m. Monday, the 53-year-old female driver of a 1990 Ford was cited for failure to yield after the stop in the 8600 block of Teal Street after colliding with a 2006 Chevy driven by a 25-year-old woman.

Theft

• At 1:40 p.m. Monday, a 23-year-old man reported a theft from a vehicle in the Juneau area.

• At 1:58 p.m. Monday, investigation continues into a report of a theft of merchandise in the 3000 block of Vintage Boulevard. Alcohol was involved.

Trespassing

• At 3:03 p.m. Monday, a 16-year-old boy was arrested for trespassing in the 1200 block of Glacier Avenue, and he was taken to Johnson Youth Center.

Warrant arrest

• At 3:25 p.m. Monday, Alaska State Troopers in Juneau arrested Michael Squartsoff, 52, on an outstanding warrant for petition to revoke probation, and he was taken to Lemon Creek Correctional Center.
